In short: Buttock pain and sciatica-like symptoms from a tight piriformis muscle irritating the sciatic nerve.

What is Piriformis Syndrome?

The piriformis muscle lies deep in the buttock near the sciatic nerve. When it is tight or in spasm, it can irritate the nerve and mimic a disc problem.

Common symptoms

  • Deep buttock pain
  • Pain sitting
  • Leg tingling

What causes it?

  • Prolonged sitting
  • Running
  • Wallet in back pocket

How it is diagnosed at Yonsei Leaders

Examination; MRI to exclude a disc.

Non-surgical treatment options

  • Manual Therapy: One-to-one hands-on treatment by a trained therapist to release tight muscles, mobilise stiff joints and correct posture and alignment.
  • Trigger Point Injection: Small injections into tight, tender knots in muscle that refer pain elsewhere, used for myofascial neck, shoulder and back pain and tension headaches.
  • Ultrasound-Guided Injection: Real-time ultrasound (E-CUBE 8 DIA) lets the doctor see tendons, ligaments, bursae and nerves while injecting, for accurate soft-tissue and joint…
  • Stretch and Spray Myofascial Release: A vapo-coolant spray combined with targeted stretching to release tight muscles and fascia and break the spasm–pain cycle.
  • Vibration Therapy: Targeted vibration energy that loosens knotted muscle and soft tissue to improve pain and movement.

Your plan is tailored after examination; many patients combine a targeted procedure with manual therapy and exercise for longer-lasting results.

Self-care tips

  • Piriformis stretches
  • Avoid long sitting
